You will get a free consultation with our professionals and should expect a 2-hour meeting where we go over all corresponding needs, desires, and problems. We assess patients’ and families needs and preferences to develop a personalized care plan. These plans are different and unique for each family and will have individualized goals. Your caregiver will start providing compassionate professional care according to your personalized plan.
This can start the same day as our consultation and look exactly like what your family needs. After implementation, we use specific tools and various metrics to monitor their ongoing health and strive for better outcomes. We keep our families informed and involved without overwhelming them. We regularly review your progress and adjust the care plan to ensure exceptional ongoing support.
